Our website does not create a patient portal and is not a secure channel for protected health information. Please do not submit symptoms, diagnoses, medication details or other clinical information through any form on this site.

Website forms collect only the contact details you enter so our staff can return your request. We use standard web analytics to understand which pages are useful; this does not identify you as a patient.

To ask a question about this notice, request records, or file a complaint, call our office at 540-444-5670 and ask for the Privacy Officer. You may also file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
